Abstract

Diabetes melitus merupakan penyakit metabolik yang umumnya ditandai dengan peningkatan kadar glukosa darah, atau hiperglikemia. Hiperglikemia berat menyebabkan gejala klasik seperti poliuria, polidipsia, kelelahan, penurunan kinerja, dan penurunan berat badan yang tidak dapat dijelaskan, gangguan penglihatan dan kerentanan terhadap infeksi, serta ketoasidosis atau sindrom hiperosmolar non-ketoasidosis dengan risiko koma.  Puskesmas (PKM) sebagai garda terdepan pelayanan kesehatan primer, memerlukan dukungan dalam mengoptimalkan program pencegahan dan pengontrolan gula darah pada pasien DM.  Edukasi dan Cek Gula Darah (EGD) merupakan metode yang diguanakan dalam kegiatan pengabdian kepada masyrakat ini, bertujuan untuk mencegah komplikasi yang timbul akibat diabetes yang tidak terkontrol jangka panjang dalam upaya meningkatkan Quality Of Life penderita DM.  Pengabdian Kepada Masyarakat ini dilaksnakan di UPT PKM Mentikan Kota Mojokerto pada tanggal 20 September 2025.  Hasil kegiatan EGD yang diterapkan sangat efektif sebagai upaya peningkatan Quality Of Life Pasien DM di PKM Mentikan Kota Mojokerto. Sinergi antara kepatuhan obat dan implementasi self-management proaktif, didukung edukasi efektif, menjadi kunci utama pasien DM mencapai target gula darah optimal (terkontrol) dan meningkatkan Quality of Life.
